Application Packet Process & Instructions
Step I
- Open each Application Packet fillable pdf document listed below, fill it out and save it to your computer to have available to send to stuemployment@puc.edu once you’ve found a job. If you have any questions about how to fill out any of the forms please see below.
- Bookmark this page for easy reference on your computer.
- Find a job on campus (try using Handshake or contacting departments directly).
- Once you have found a job, fill out Section I of Form I-9 (Employment Eligibility Verification) and save it with the rest of your documents.
- E-mail your completed application packet documents as individual attachments to: stuemployment@puc.edu
- In the Subject Line put your Name – Student Employment Application (ie. Jane Doe – Student Employment Application)
Note: To submit your application packet electronically, you will need to use a signature app to add your signature/initials where required (Adobe has a free version). We cannot accept photos of your forms. They must be returned as PDF files. You must come to Student Employment in person to present your original identification document(s) for your I-9 form verification before you can begin working.
Step II
- Bring your original identification document(s) to Student Employment (Financial Services Building across from Grainger Hall). We cannot accept photos or photocopies of your ID documents – we are required by federal law to examine the originals. We will verify your document(s) to ensure it/they meets the I-9 form requirements and fill out Section 2 of the I-9. We will also check to make we have received all other required forms electronically.
- Once your application has been accepted as complete, we will send an electronic contract to the Dept. hiring you.
Step III
- Once we have received the contract back from the hiring department, we will process your application and email you to let you know that you are cleared to begin work (with a copy to the Dept. you will be working for so they know you’re cleared).
- At this point (and only at this point!) you are authorized to begin working.
- Last but not least, once you have received our email that your position has been set up, download the JBDev app so you can clock in and out on your phone instead of on the wall clocks around campus with your ID card (see instructions). If you prefer to use the time clocks around campus, bring your ID card to our office and we will code it for you.
Instructions for Completing Forms
Application Form
This is the general application form for all jobs at PUC. Fill out as much as you can, and make sure you initial each paragraph and sign and date it on the last page.
Employment Handbook and Signature Page (complete only after reading the Handbook)
Read through the handbook. (Student Employment Handbook link) This is so you know what to expect as a student worker and what is expected of you. Once you have read the Handbook, complete the Signature page to return with your application packet documents.
Confidentiality Agreement
A lot of jobs on campus require confidentiality. Please print your name at the top on the first page on Page One where indicated in the first paragraph, read the agreement, then sign, date, and print your name at the bottom. Your title is student worker.
Student Employment Information Form
This form provides us with contact information for you and your wishes related to voluntary deductions. Complete the general information in the top section and circle yes or no for the questions on the bottom. If you are an international student, it is very important that you mark yes on the first yes/no question.
FERPA form
This is the Family Educational Rights & Privacy Act form. As the form explains, FERPA is a federal law designed to protect the accuracy and privacy of education records. It provides guidelines for appropriately using and releasing student education records. By signing it, you are agreeing that any confidential information you are exposed to while working at PUC, you will not share with anyone not qualified to know. Read through it and sign the second page. Make sure to print your name, write your PUC ID number, sign on the signature line, and date it.
Direct Deposit Authorization
Having Direct Deposit set up simplifies receiving your pay as it will be transferred directly into your bank account. In order to set up your direct deposit, we need 1) the Direct Deposit Authorization form, 2) a voided check or 3) a form from your bank showing your bank routing and account number.
If you have your bank’s app on your phone, that shows your routing # and account #, you can show that to us when you bring in your I-9 form ID in lieu of #2 or #3. NOTE: On the Direct Deposit Authorization form, the bank address is the location of your home branch.
Form DE 4
This is the State Tax form that determines how much state tax will be deducted from your paycheck. Fill out the personal information, then the tax information. If you want to claim allowances, you cannot claim Exempt and if you claim Exempt you cannot claim allowances. If you check one of the boxes to claim Exemption AND put a number on one of the lines for allowances, you will need to fill out a new form doing only one or the other. You will need to decide which you qualify for and fill out the form accordingly. Finally, make sure you sign and date in the middle of the page.
Form W-4
This form determines how much federal tax will be deducted from your paycheck. Fill out the information on the front page according to the form’s instructions. Don’t forget to sign and date. International Students: See IRS Notice 1392
Form I-9
Do not fill out until you have been offered a job. This form verifies your eligibility to work in the United States.
On page 3 is a list of acceptable documents to verify your identity. You can present either one item from List A, or one item from List B and one item from List C. We are required by federal law to examine the original documents. We cannot accept photocopies, pictures, or view them electronically.
If you are not a US citizen, read page 3 carefully to determine what you need to provide as authorizing identification.
Once you have been offered a job:
- Fill out the front page ONLY
- Check off the box indicating whether you had assistance completing the form or not
- Sign and date
- Save and submit to Student Employment with your other application documents
- Visit Student Employment to present your original ID documents to verify your I-9 form
