Concern

The Care & Concern form is available to report a concerning incident or ongoing concerning behavior. This form may be used for either students or employees of the College.  Reports will be automatically routed to the appropriate office for response.

 The Title IX form is available to report sex and gender-based discrimination and sexual harassment (including sexual assault, dating violence, domestic violence, and stalking), and other prohibited conduct as outline in the College’s Sex Nondiscrimination and Sexual Harassment Policy.

The Harassment and Discrimination:  Protected Classes form is available to report incidents of harassment and discrimination on the basis of race, color, national or ethnic origin, age, disability, or prior military service as well as retaliation for reporting or participating in an investigation regarding a report.  For further information, see the College’s Harassment, Discrimination, and Retaliation Policy and Grievance Process.
